Required Documents for Admission
- Referral Form No. 7 from the referring medical facility under Clinical Pathway 256 and Clinical Pathway 254. Admission must take place on the day of discharge from the referring facility.
- Personal outpatient card
- Previous medical reports and test results from laboratory, microbiology, imaging diagnostics, and others
- Prescribed medications currently being taken
Essential Personal Items
- Toiletries
- Pajamas/nightgown – appropriate for the season
- Underwear
- Slippers
- Wet wipes
- Diapers or incontinence pads – if needed
